About this assessment
The SCOFF questionnaire is a five-question check used by doctors to notice when eating habits might be causing distress. It's quick, and there are no right answers.
Before you start
- Answer yes or no to each question.
- There are no right or wrong answers — go with your first instinct.
- Your answers are saved to your account. You decide whether to share them with your therapist.
This is a screening questionnaire, not a diagnosis. If you’re in crisis, get immediate support.
